The computer is a Dell desktop with 2 USB plugs in the back and 1 in the
front.

The rear slots contain:
(a) Optical Mouse
(b) A 4-slot hub (contains my DVD-RW, external Hard Disk, Scanner, and USB
extension cable). tje DVD-RW, HD, and Scanner are usually turned off unless
needed.

The front slot is empty.

This configuration has existed for the last year or so without problems.

I noticed the "lost" the icon about 2 weeks ago.
